Enter your team's official name as well as the complete contact details of the team representative (email and phone number). This information will be used to contact you if your team is selected.

Highlight your team's achievements and community involvement with a short but inspiring text (maximum 500 characters)

3. Capture the moment
Submit a team photo that showcases your passion and team spirit.

4. Get authorization
Make sure you have parental consent for each player featured in the submitted photo.
